Evidence

Claims should not outrun the evidence.

Enterprise Reliability combines structured interviews with operating evidence. Each finding explains what we observed, what it may mean, the risk it creates and how confident we are.

EVIDENCE RULE

Important findings normally require support from more than one independent source.

EVIDENCE MODES

Triangulate the operating reality.

No single interview, dashboard or document is treated as the whole system.

01

Structured interviews

Role-specific conversations with executives, sponsors and selected stakeholders.

02

Operating evidence

Relevant decision records, commitments, forecasts, governance materials and operating artifacts.

03

Decision reconstruction

A trace of selected consequential decisions from signal and assumption through authority, commitment and execution.

04

Pattern corroboration

Comparison across independent evidence modes, including contradictions and exceptions.

RISK ASSESSMENT

Turn evidence into a useful finding.

Each material issue is described in a way that supports a measured response and avoids sweeping judgments.

RESEARCH BASIS

Grounded in established fields, applied conservatively.

The discipline draws from systems theory, high-reliability organization research, decision science, organizational design, cognitive-load research, behavioral psychology, stress physiology and performance under pressure. These traditions help explain why capable systems can become unreliable when demand, information, authority and recovery conditions change.

The assessment does not convert those research traditions into clinical claims. Reliability Margin is a structured risk indicator, not a physiological measurement, validated predictive score or promise of mathematical precision. Language is calibrated accordingly: evidence may suggest, support or increase confidence in a finding; it does not create certainty where certainty is unavailable.

Contradictory evidence is retained. Anonymous identifying quotations are avoided. Findings distinguish observation, inference and conclusion so sponsors can evaluate both the issue and the basis for it.
